Position Description
This position will serve as primary contacts for medically needy patients. Responsibilities include processing provider claims, using computer system to record and report patient information, patient case finding, financial screening, program enrollment facilitation, non-medical counseling, case tracking, and preparing all necessary case documentation. Persons in this position will serve as program spokespeople and representatives to participating health care providers, local communities and local health agencies; therefore, this position requires a positive and professional attitude when working with patients, community leaders, and local health agency and state agency care workers. Persons in this position will be expected to travel between local health-agencies in the fulfillment of their duties and may be asked to make occasional home visits. Work is performed in usual clinic conditions; requires a high degree of contact with patients and families, clinic staff and outside support agencies. Travel and ability to respond to needs during off- duty hours may be required in unusual situations.

Major/Essential Functions
  • Engage in community outreach. Educate, navigate, and provide obesity prevention outreach in El Paso County residents. 
  • Provide health education on obesity, nutrition information, physical activity, health meal preparation, and Cancer Prevention. 
  • Recruitment of program participants. 
  • Organize and maintain documents on educational sessions and participant contacts. 
Position Specific Qualifications
Ability to assist clients, find resources to increase physical activity and better nutrition. Willingness to work with community agencies to promote obesity prevention programs. Travel to various parts of El Paso County to deliver education programming. Provide exercise and cooking demonstrations to small groups.

Preferred Qualifications
The ideal candidate will be bilingual (Spanish/English), have knowledge and experience in community health education, and have experience in public speaking. Familiarity with community engagement, obesity prevention, healthy lifestyle, and nutrition education. Ability to work independently with minimal supervision, ability to use computers, tables, smartphones or similar technologies.

Required Qualifications
