Position Description
This position is responsible for assisting patients and their families with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care; or may also be responsible for working with patients who are emotionally or mentally disturbed, collaborating with psychiatric and allied provider’s teams in diagnosis and treatment plan.
They work closely with physicians and nursing staff to further their understanding of significant social and emotional factors underlying patient's health problem. Work is performed under the general direction of the Director of Social Services, and requires frequent contact with representatives of other social service related agencies.

Major/Essential Functions
- Establishes rapport with individuals in targeted communities that are being asked to participate in the project. Work with the TTUHSC El Paso department of Psychiatry, community organizations such as the opportunity center. Inform individuals about the ability and resources for mental health. Must meet established goals and deadlines. Is expected to meet with about 20-30 participants per week.
- Attend weekly meetings to report back to the team on the outreach initiative and update material about patient navigator. Must be available to attend meeting: and be prepared to discuss as instructed by Project Director.
- Collect data using Microsoft surfaces and report back to the community engagement earn. Work with the team to document recruitment and education activities. Data collection must be organized, consistent, accurate and readily available for review.
- Maintain confidentiality per HIPAA guidelines in regard to participant information. Performs all other related duties, within the scope of practice, on an as needed basis or as required by Supervisor. Must meet established deadlines and follow all TTUHSC policies and procedures. Report any concerns to Program Director immediately.
- Collaborate with community agencies and providers to ensure access for service in line with Individual Treatment Plans. Maintain positive community relationship to ensure accurate referral access. 100% of all services will have proper authorization on file.
- Other duties as assigned Employee Is responsible for performing all other relates duties, as assigned. Standard of Expected Performance. Must adhere to deadline and criteria given by the person assigning the task to ensure completion.
- Communication: Communicates respectfully with team members and supervisor in a timely manner utilizing direct, complete, and adequate information. Communicates with outside social service and government agencies in a professional manner, upholding and adhering to privacy standards on behalf of the patients. Communicates effectively and productively with patients family members as needed and permitted based current policies. When problems arise they are addressed directly with the person(s) involved or speaks with their supervisor exclusively. Receives and is open to constructive feedback. Seeks feedback from others to improve clinical practice and safety. As evidence by expressed, demonstrated and written evidence of direct, respectful, complete and adequate communication with colleagues and supervisors.

Required Qualifications
- A minimum of a Master's degree in Social Work from an accredited Graduate School of Social Work.
- Licensed as a Social Worker by the Council on Social Work Education.
