Position Description
The case manager serves as counselor and resource person for a caseload of patients. Specific duties include: providing individual and group counseling; participating as a member of a treatment team; corresponding with outside agencies, institutions and families of patients; collecting and writing social histories; monitoring patient/client treatment plan reviews; and compiling staffing notes. Functions under the immediate supervision of a supervising psychologist, social worker, or physician.

Major/Essential Functions
  • Assumes the role of counseling therapist for individuals, couples and families with psychiatric and medically referred issues and health related problems, such as chronic illness, psychosomatic illness, adjustment disorders
  • Coordinates care with medical providers, coordinate community service resources with patient needs
  • Consult with medical providers regarding mental health interventions with patients
  • Perform urgent mental health assessment and consult
  • Management of patients with personality disorders
  • Develop, maintain and lead the behavioral health curriculum
  • Coordinate, maintain and lead behavioral health curriculum
  • Coordinate the psycho-social topics in the conference schedule
  • Teach or facilitate teaching of behavioral and mental health topics relevant to board reviews
  • Performs other tasks and duties assigned by supervisor within the policies of the department and TTUHSC in order to meet the goals and objectives of the program and the department

Required Qualifications
  • Bachelors degree in Social Work and 4 years of related experience OR Masters degree in Social work and 2 years of related experience
  • Credentialed as a Certified Social Worker
