Position Description
The case manager serves as counselor and resource person for a caseload of patients. Specific duties include: providing individual and group counseling; participating as a member of a treatment team; corresponding with outside agencies, institutions and families of patients; collecting and writing social histories; monitoring patient/client treatment plan reviews; and compiling staffing notes. Functions under the immediate supervision of a supervising psychologist, social worker, or physician.

Major/Essential Functions
  • Conducts case management services for the BCCS Program.  Conducts comprehensive patient assessments and develops service plan.  Screens patients and submits applications for the Medicaid for Breast and Cervical Cancer Program.  Assists with renewal and reinstatement process for Medicaid for Breast and Cervical Cancer as needed.
  • Maintains case records and documents all case activity.  Maintains billing records.
  • Provides social service assistance and counseling to patients of the Ob/Gyn clinic.  Refers to community agencies as needed. Functions as part of a treatment team.
  • Serves as backup for screening and enrollment of patients for the BCCS Program.  Serves as backup for assisting clients in completing application for the CHIP Perinate Program, the Healthy Texas Women's Program and the Arch Program.  Screens and enrolls patients in the Family Planning Grant.
  • Maintains statistical data and compiles reports on case management services for performance improvement activities.  Audits patient records for eligibility, clinical and billing compliance.
  • Maintains case staffing with supervisor as needed.  Is assigned as the safety officer for the social service area and serves as essential personnel during inclement weather.
  • Serves as back-up to the Director of Social Services in her absence.
  • Conducts screenings for potential abuse situations.  Reports any suspected cases to the State.
  • Must maintain an updated community resource booklet.
  • Schedules sign language interpreter services.

Required Qualifications
Education/Experience:
A Master's degree or equivalent OR at least four years related experience and/or training OR equivalent combination of education and experience.

License/Certification:
Credentialed as a Certified Social Worker.
